PAYMENTS: Payment is due at the time of the visit. Preferred forms of payment are cash and check. Credit card payment is accepted. Lower payment is considered due to financial hardship.
INSURANCE: We do not bill insurance.
Initial Visit
Please bring the following:
. I.D card
. A copy of recent lab results, imaging reports and medical records, if available.
. All medications and supplements
. Completed Patient Profile (it will be sent to patient via e-mail when making an appointment).
We will discuss your present health concerns, pertinent physical examinations, and potential laboratory recommendations.
Office visit: Case Analysis, Assessment and Individualized Treatment Regimen
We will go over a summary of case analysis based on your past and recent medical records, lab results and imaging reports, diet, lifestyle, etc.; assessment with further evaluation with specific workup; and an individualized treatment regimen. Physical examinations that pertain to your health concerns are included.
